How to register
Welcome to Friarsgate. We are currently able to take on new patients at all three Winchester based surgeries:
Friarsgate, Weeke
Badger Farm
Kings Worthy
To register with us please download and print both PDFs below. Fill them in, sign them and bring them into your chosen surgery with the following documentation:
- A proof of identity, e.g. drivers license or passport
- A proof of address, e.g. recent utility bill or bank statement
- If you are returning from the Armed Forces, your discharge papers
Please note that we are unable to accept registrations via the post, as we are required to check ID with each patient in person.
On registration, you will receive a practice booklet and confirmation of who your doctor will be if you have not stated a preference.
