Appointments

The surgeries run on an appointment system. When you contact the surgery the Care Navigation team will ask you some information about what is wrong with you so they can sign post you to the most appropriate clinician or service.

Appointments can be broadly broken down into 4 main areas:

GP Appointments

We encourage our patients to see or speak with their own GP. When contacting the surgery you will be asked for some medical information by the care navigation team team to allow them to sign post you to the most appropriate service. Your GP can offer both face to face and telephone appointments. The majority of clinicians appointments are of 10 minute duration.  We ask that you try to observe the “one appointment, one problem” ethos.  We strive to deal with each medical problem to the highest standard and this proves to be impossible if we are asked to deal with a number of problems in one appointment slot.

 

Nurse Appointments

There are many procedures and conditions for which it is not always necessary to see a GP, but instead one of our fully qualified nurse clinicians can help. They offer appointments for Asthma, Diabetes, COPD, Sexual Health, immunisations & Contraception.

On the day care team

This is available 8am – 6.00pm Monday to Friday.
If you have a medical problem or need advice on the day, speak with our Care Navigation team to arrange a telephone consultation with our Advance nurse practitioner led on the day team. The on the day team will be able to book an appointment that day if needed or with a clinician when appropriate.

Clinic Appointments

We are expanding our multi- disciplinary team and are proud to offer our patients appointments with a range of new specialists such as our physiotherapists, pharmacists and a range of appointments with our Primary Care Mental Health Team.

Making an appointment

The surgeries run on an appointment system. When you contact the surgery the Care Navigation team will ask you some information about what is wrong with you so they can sign post you to the most appropriate clinician or service. Routine appointments for GPs can be booked a maximum of 3 weeks (21 days) in advance, with appointments being released on a daily basis. You can book an appointment in 3 ways:

Online – register with Online Access to book appointments, order prescriptions and manage your patient record

Telephone the surgery on 01962 871730 and speak with our Care Navigation Team

Drop in to either of our surgeries and speak with the Reception Team in person

Online Access

This service allows you to book & cancel appointments, order prescriptions and view some of your medical record online. To register for the service you will need to visit the surgery with 2 forms of ID, one of which must be photo ID. You will then be asked to complete a registration form which will be processed and login details issued. Once this is set up, you can access this any time via the website.

On the day team

Between the hours of 8:00am – 6:00pm Monday to Friday we operate a On the day Triage Team system at the Weeke surgery. If you feel you have an urgent medical  problem or need advice that day, you will be given a telephone consultation with our Advance nurse practitioner led duty team. The on the day team will be able to arrange an appointment that day or with your own GP or with another clinician when appropriate.
