How to register as a new patient

We are pleased to accept registration requests from patients who are living within the area indicated in the map below:

If you are new to the UK or have never registered with an NHS GP in the UK previously we will need two forms of ID, please upload these documents when completing this form.

We ask all new patients to complete our online registration questionnaire (see links below).  If you do not have access to the internet at home this can be done in practice. When a patient registers with a Practice it can take some time for medical notes to be transferred from the previous practice so it’s very important for the form to be completed accurately so that we can monitor any conditions appropriately.

There are a number of reasons why you may not be able to register with your choice of GP, for example you may live too far away.  If this is the case please visit www.nhs.uk to find information on GP practices in your local area.

NHS Out of Area Registration 

As a practice we have decided not to participate in the NHS Out of Area Registration process that was launched in Jan 2015 as we do not believe it’s operationally safe or in the best interest of the registering patient. Our practice ethos is that you are assigned a registered GP who takes ownership and responsibility for your care and we would not want to delegate any responsibility to another provider to respond to your urgent home visit request/s. Our local LMC and GPC are questioning whether the launch was rolled out correctly and whether systems are robust enough to provide a safe and adequate home visiting service.

We have the right to refuse to register patients if they have a violent flag against their record.
